I've heard of instances where performing certain types of header and
body checks at the upstream MTA can cause the connection to amavisd-maia
to drop prematurely when Postfix rejects mail during the DATA phase of
the SMTP transaction; that may be what's happening here. If you're
using header_checks or body_checks at your upstream MTA, remove those
tests--they're redundant anyway, since SpamAssassin does the same sorts
of tests.
