Schedule the process-quarantine script to run once per hour.  It may
take more than an hour to finish at times, but that's fine--it won't
start duplicate processes if one is already running, and it will check
for any remaining work to be done before it terminates, so items that
get confirmed while process-quarantine is running will get handled
before it exits.

As for the system load issue, that's a much bigger one to address, but
as a starting point, try to determine where the bottlenecks are.  Some
recent posts here explained how to do that by setting $log_level to 1 or
higher in amavisd.conf (so that you can see the timing breakdown at the
end of every processed mail item), and by using "spamassassin -D -t <
email.txt" to look for bottlenecks within SpamAssassin.  Before you go
disabling tests blindly in the hope of lessening your load, check these
things out.  You may also want to look at whether you're running the
optimal number of amavisd-maia children to handle your load on your
hardware.
