One common cause of "MySQL has gone away" errors is extended inactivity
(i.e. timeouts).  This is particularly common in test environments,
where the server is not exposed to a regular flow of traffic.  In that
case, amavisd-maia may go for many (6-8) hours without receiving any
mail to process, so the database connection times out, and then the next
time a mail item arrives to be processed, you get the "MySQL has gone
away" error.  The problem corrects itself, however, in that the item
remains in the upstream MTA's queue, and goes through properly the next
time through (since the database connection is reestablished at that
point).  In that sense it's a harmless error, and one that's rarely (if
ever) seen in production environments.
