Have you adjusted the GRANT statements in the database to allow your
amavis/maia user to connect to it from all of the hosts that need to
communicate with it?  The default instructions assume you're running
everything on a single host, and so it grants permissions to
"amavis at localhost".  If the database is on a separate machine you'll
have to add a similar GRANT statement for "amavis at web.server.host" and
possibly "amavis at amavisd-maia.host".  The fact that one of your
amavisd-maia hosts can connect with the database and the other can't
hints strongly at a GRANT issue.
